The Jacksonville Urban League Head Start Program is now accepting registration for children *3, 4, and 5 years old for the 2006-2007 School Year.

Proof of all Family Income (W-2 & current check stubs)
TB Test and Results
Immunization Record (680 Blue Card)
Medical Insurance Card (Medicaid or Private Coverage)
Physical
Social Security Cards (All Family Members)
Guardianship Documents (If applicable)
