The Jacksonville Urban League Head Start Program is now accepting registration for children *3, 4, and 5 years old for the 2006-2007 School Year.

Documents Required for Registration

• Birth Certificate
• Proof of all Family Income (W-2 & current check stubs)
• TB Test and Results
• Immunization Record (680 Blue Card)
• Medical Insurance Card (Medicaid or Private Coverage)
• Physical
• Social Security Cards (All Family Members)
• Guardianship Documents (If applicable)
